約在兩晉時期（265—420），嗩吶已在新疆拜城一帶（古稱龜茲）流傳，新疆克孜爾千佛洞第三十八窟（開鑿於公元三世紀）壁畫中有其圖象（《中國音樂史圖鑑》Ⅱ—74至Ⅱ—75）。北魏時期（386—534）開鑿的山西大同雲崗石窟第十洞也有演奏嗩吶的伎樂人雕刻。這種嗩吶的哨、管身和喇叭碗成為一體，可能是其早期形態，與現代新疆維吾爾族木嗩吶相近。直至明代，中原地區使用的嗩吶，其結構則與現代嗩吶相同。明徐渭《南詞敘錄》、王磐《王西樓先生樂府》、戚繼光《紀效新書·武備志》均有記述。明王圻《三才圖會》述其形制：“其制如喇叭，七孔；首尾以銅為之，管則用木。不知起於何代，當是軍中之樂也。今民間多用之”。清代，嗩吶又稱金口角、蘇爾奈。在宮廷中列入回部樂，並用於燒歌大樂。《律呂正義後編》、《大清會典》、《皇朝禮器圖式》均註明“蘇爾奈，又名嗩吶”，並附圖說明。在民間已廣泛應用於器樂演奏、戲曲伴奏和婚喪喜慶活動。
嗩吶由杆、哨、氣牌、侵子和碗構成。杆用紅木製成，為空心圓錐體，上開八個（前七後一）圓形音孔，表面旋成竹節形狀，取其美觀和按孔方便。哨蘆葦制，一頭扁平狀為簧哨，另一頭圓形纏銅絲插入侵子。氣牌為兩塊圓形銅片、有機玻璃或賽璐珞片，分上下套於侵子上，中間夾以葫蘆形裝飾物，上片用以托住嘴脣，下片壓緊杆身，以邦助運氣、使口力持久。侵子為銅製圓錐形細管，上插簧哨，下與管身相接。碗又稱喇叭口，用薄銅片製成喇叭形，套於杆下端，可活動和裝卸。
品種較多，依杆的長短分為海笛、小嗩吶、中嗩吶、大嗩吶和柏木杆五種。杆身全長18.2—31、下口內徑1.8—2.65、下口外徑2.4—3.45釐米。碗高7.1—13.6、上口內徑2.35—3.25、下口直徑8.6—14.4釐米。海笛杆短碗小，發音尖銳，音色高亢，用於器樂合奏。流行於江、浙、皖等省；小嗩響音色柔和，常用的七寸杆，又稱三吱子。用於獨奏、合奏或歌舞伴奏，流行於兩廣、湘、閩、贛等省、區；中嗩吶音色較柔和，常用一尺一寸杆，又稱黑杆子。流行於江、浙、皖一帶。困上述嗩吶流傳於南方各省，故稱南方嗩吶。大嗩吶發音低沉，常用一尺五寸杆，又稱大杆子或二混頭。流行於東北、山海關和冀東一帶；柏木杆發音清脆明亮，用以吹奏小調和咔戲（模仿戲曲唱腔）。大嗩吶、柏木杆流傳於北方各省，故稱北方嗩吶。各種嗩吶音高不等，常用的有D調小嗩吶，G、F調海笛，A、bB調中嗩吶和大嗩吶。常用音域兩個八度。
嗩吶常作為領奏樂器或與鑼鼓結合演奏，宜於表現熱烈、歡騰的氣氛。在農村用於集會、豐收、節日慶祝或婚喪儀仗；是河北吹歌、山東鼓吹、遼南鼓吹、潮州大鑼鼓和山西八大套的主奏樂器。
演奏技巧有連奏、單吐、雙吐、三吐、彈音、花舌、簫音、滑音、顫音、疊音和墊音等，還可模仿飛禽鳴叫聲。傳統曲目多源於民歌、地方戲曲、民間小曲和戲劇曲牌。樂曲有《百鳥朝鳳》、《小開門》、《一支花》、《鳳陽歌絞八板》、《婚禮曲》和《小放牛》等。
